Job Description

We seek a highly motivated and experienced professional to join our multidisciplinary team. The ideal candidate will excel in coordinating care, resources, and services for our members to achieve optimal clinical and resource outcomes.

What You’ll Do

  • Utilize applicable clinical expertise to perform thorough assessments and collaborate with stakeholders to develop comprehensive care plans.

  • Develop member-centric plans tailored to individual needs, health status, educational status, and level of support needs, while identifying and addressing barriers to achieving goals.

  • Utilize community resources and funding sources as necessary in developing care plans.

  • Perform ongoing monitoring and management of members, including scheduled follow-ups, discussions of plans, provision of appropriate services and education, referrals, and assessment of progress towards goals.

  • Evaluate and facilitate care provided to members across the continuum of care, from physician offices to hospitals, rehabilitation units, skilled nursing facilities, and home care.

  • Educate members and promote proactive interventions to limit expenses and encourage positive outcomes.

  • Effectively document all aspects of care plans, from initial assessment to development, implementation, monitoring, and evaluation of outcomes.

  • Conduct outreach to members identified as high-risk, high-cost, or high-utilization cases.

  • Review alternative treatment plans for case management candidates, assess available benefits, and determine the need for benefits exceptions or flex benefit options.

  • Evaluate medical necessity and appropriateness of services as defined by the department.

  • Develop relevant policies, procedures, education, or training materials for internal and external use as needed.

You will need to have:

  • Strong knowledge of community resources and funding sources.

  • Excellent communication, interpersonal, and organizational skills.

  • Ability to work collaboratively in a multidisciplinary team environment.

  • Proficiency in documentation and record-keeping.

  • Experience in developing policies, procedures, education, or training materials preferred.
